Updates to COVID-19 Protocols As of June 13, 2022 - Please do not arrive more than 5 minutes ahead of your scheduled appointment time. - Patients may use their own discretion with respect to wearing a mask. You may request that your healthcare provider wear a mask for the duration of the appointment. - Please respect the choice of other patients with regard to mask wearing. - Patients are required to self-screen for COVID symptoms before entering the building. Please reschedule if you have symptoms of COVID-19, or if you or a member of your household has tested positive.
Updates to Our Missed Appointment Policy: as of April 1st, 2022, any missed appointments or late cancellations (less than 24 hours’ notice) will be billed the full fee for the visit. This fee is not eligible for coverage by third party insurance and is the responsibility of the patient.
2022 Orthotic Deadline: To ensure we receive your order to dispense to you for eligibility under 2022 benefit claims, we must receive your order request & a valid prescription no later than Friday, December 2, 2022. Any orders/Rx received after this date will be dispensed in Jan 2023 and therefore eligible under 2023 benefit claims.
